10 What should I do if I forget my User Name and/or Password?

If you forget your User Name and/or Password, it can be retrieved online by returning to the MDUFA User Fee website and clicking on Forgot your Username/Password? link.  User will need to enter either their User Name or Email Address only, and then click on the 'Email My Password' button.  An e-mail will be sent to the user with the requested information.

If your account is locked, contact the User Fee Helpdesk at (301) 796-7200 or email userfees@fda.gov to retrieve your login information.

11. What should I do if information associated with my organization changes?

You can edit the general information, including contact name and/or billing address by going to MDUFA User Fee website.  Login with your Username/Password then select the 'GO' button for the MDUFA User Fee cover sheet option and then click the'Profile' icon at the top of the page. However, to edit your organization name, Employer Identification Number (EIN), or DUNS number email the User Fee Helpdesk at userfees@fda.gov with your current and new updated information.  Please be sure to provide your FDA assigned Organization Number on your Profile page with your request.
